DIRECT PRIMARY CARE

A Better Way to Receive Medical Care

Direct Primary Care is open and we are taking new patients ages 14 and up and older in Silverton and nearby Willamette Valley communities. This care model is built on strong patient–provider relationships, emphasizing trust, compassion, and open communication.

By focusing on education, prevention, and personalized care, patients are empowered to make informed decisions about their health. Care plans integrate current medical advancements with holistic therapies to support comprehensive, individualized treatment.

Direct Primary Care moves away from rushed appointments, fragmented care, and unclear costs—offering a more personal and transparent healthcare experience. Pre-booking will be available soon for those seeking relationship-driven primary care in Silverton and the surrounding area. Updates will be shared on the website and social media.

Patients may book appointments by clicking on booking link. Choose a membership plan and book “NEW patient appointment. Either “monthly” or “yearly” membership plan. $75 enrollment fee will be charged at time of enrollment with membership plan charged at time of first “welcome” visit.

DPC Plan 1

Monthly Membership

$75 Enrollment Fee

Beginning on Welcome Visit and charged monthly thereafter at the end of the month

Services include:

  • 20 in office visits a year, with 1 set of preventative health labs included during yearly wellness visit.

  • Discounted labs will be offered if needed and cost will be transparent.

  • Discounted cash pay imaging will be offered if imaging is necessary. Telemedicine appointments will also be available to patient if they are appropriate.

  • Patients will have direct access to their provider through texting, phone calls and emailing.

  • Same day or next day appointments will be available for acute health concerns.

  • Follow up calls will be made within 24 hours for non acute issues.

DPC Plan 2

Yearly Membership

$75 Enrollment Fee

Yearly membership fee will be charged at welcome visit.

Services include:

  • Patients will receive 6 yearly in office visits, may request telemedicine visits if deemed appropriate by the provider, labs will be offered at a discounted rate, imaging at a discounted rate if either are needed for acute issues.

  • Yearly wellness exam labs must be paid by patient at a discounted rate. Patients will have direct access to their provider through text, phone and emailing. Same day or next day appointments for acute health concerns

  • Follow up calls will be made within 24 hours for non acute health concerns

  • This plan is designed for patients who want the convenience of quicker access for acute concerns but who do not visit the clinic regularly.
